Our advocacy service can help make sure your senior clients are getting the care they need.
Our Health and Well-Being program involves setting up support services, managing those support services, and a weekly check in visit. This service will evaluate the client’s needs on an ongoing basis and recommend additional services as needed, or to discontinue a service if no longer needed. This allows your loved one to remain as independent as possible at home.
The program is catered to those family members who live far away or are overwhelmed with their own family and professional demands. Thriving at Home can be the eyes and ears for you when you are away, in turn allowing you to have quality “one on one” time with your loved one when you are together.
Our goal is to save our clients and their families time, stress and money by navigating and vetting the multitude of aging services available to them.
Senior Advocate
Thriving at Home also offers Senior Advocacy. We can accompany clients at medical or other professional appointments and thoroughly document the proceedings, help them understand the outcomes, and follow up on any action items. This information may also be communicated to relevant family members to keep everyone informed. We can also assist in navigating the health care system, providers and bills, veteran’s benefits, Medicare, and insurance reimbursement resolution.
With this program, weekly written reports detailing information about the client and services will be provided. We will be the eyes and ears for the Adult Child. Additional hours can be added to the monthly support services management as needed.
